Informed Consent DRAFT*WHAT THIS SURVEY IS ABOUT
You are invited to answer a few questions about how well the fall semester is going for you and if you would like any assistance. Your participation is voluntary and confidential. Information from the survey is used by your advisors, faculty and other staff who provide student services at your institution. An academic advisor or other staff may reach out to you regarding your responses.
SURVEY PARTICIPANTS
Survey participants are new entering students at your school, as well as at other colleges and universities.
TAKING THE SURVEY
The survey asks you about how you have been doing since the start of the academic term. It is very short. Your participation is completely voluntary. Declining participation or not completing the survey will not result in any penalty or loss of benefits at your institution.
SURVEY PARTNERS
This survey is conducted in a partnership between your school and the Indiana University Center for Postsecondary Research. The Center will provide your survey responses to your school. Someone from your school may reach out to you only if you requested additional information.
CONFIDENTIALITY
Your school and the Center will make every effort to keep your responses confidential, although absolute confidentiality cannot be guaranteed. Neither your college nor the Center will make any public release of information associated with your name while using survey results and related student records for their intended purpose (to improve the college experience and conduct research), but personally identifiable responses may be inspected by college and government organizations when required by law.
